Dark circles are having a moment as a sign of low iron, so here is what ferritin actually does and why your reflection makes a poor blood test.

Every few weeks the internet finds a new bodily feature to worry about, and this month it is the skin under your eyes. "Ferritin face" is the phrase doing the rounds: the idea that dark circles and a permanently shattered look are outward signs of low iron stores. There is a kernel of sound physiology in there, which is more than most viral health claims manage. It has also been stretched into something it cannot carry.

What Is Ferritin?

Ferritin is your iron savings account.

Some of your iron is out doing the day job in your red blood cells, moving oxygen around. The rest sits in reserve, held by ferritin, an iron-storage protein present in all your cells. It releases iron when your intake falls short of what you are spending.

Here is the part worth knowing. When you run a small daily shortfall, your body does not let your red blood cells take the hit first. It dips into the savings. Ferritin is therefore the earliest number to move, and it can drift down for a long stretch while everything else on a blood test still looks unremarkable. Low ferritin concentrations suggest the stores are thinning, before anything more dramatic happens.

Which is why you can be running short on iron for a good while before it shows up anywhere else on a blood test.

That is the legitimate idea buried inside the trend. Ferritin tells you about the reserve, not the current account.

So Do Dark Under-Eye Circles Mean Low Iron?

Usually not. This is where the trend overreaches.

Dermatology has a name for dark circles, periorbital hyperpigmentation, and it has been studied properly. A review in the Journal of Clinical and Aesthetic Dermatology lists the causes as heredity, excessive pigmentation, pigmentation left behind by eczema or allergic contact dermatitis, puffiness around the eye, prominent blood vessels close to the surface, and the shadow cast by skin laxity and tear-trough hollowing as we age. Iron does not appear on that list.

The most likely explanation for your under-eye circles is the face you inherited. How thin the skin is there, how it is pigmented, where your blood vessels happen to sit. If a parent has them, that is usually the answer. After that come the ordinary suspects: getting older, hay fever, a late night, a fortnight of late nights.

There is a plausible mechanism behind the iron version, and our Head of Nutrition, Evan Lynch, describes it like this: the skin beneath the eye is thin, blood carrying less oxygen is a less vivid red, so if reserves drop low enough the vessels underneath can read as slightly blue. Sound as far as it goes. The trouble is running that logic backwards, from a face to a blood result. Doctors commenting on the trend keep making the same point, which is that these changes can happen but are not specific to iron and are no basis for diagnosing yourself.

A symptom with thirty possible causes is not much of a symptom. If you have had faint shadows under your eyes since you were nineteen, they are not a warning light. They are your face.

What Low Iron Stores Actually Feel Like

When a mild shortfall in iron does announce itself, it does so from the inside and without much drama. The British Dietetic Association describes it plainly: you may look pale, and you often feel tired and lacking in energy. The longer, more alarming lists you will find online describe the severe end of the spectrum, not the everyday version.

Athletes tend to notice it before anyone else, as training that feels harder than the numbers justify. Whether topping up iron helps endurance athletes who are low on stores but not anaemic has been reviewed formally, and they are one of the few groups where the question has been properly asked.

The caveat you would expect: tiredness and low energy describe low iron stores, and they also describe roughly forty other things, a demanding month included. Fatigue is the least specific symptom in medicine. For the wider picture, we have written about why you might be tired all the time.

Why "Normal" Results Do Not Always Feel Normal

A wrinkle that explains a common frustration.

Because the reserve empties first, you can be low on stores while a standard blood panel comes back fine. People in that position get told they are fine, and are then left puzzled, because they do not feel it.

Where "low" starts is also less settled than you might assume. Laboratory reference ranges are wide and differ between labs. In practice a ferritin below around 30 micrograms per litre is widely treated as an indicator of iron deficiency. That proviso matters, because ferritin climbs during infection and inflammation. Where inflammation is present, the World Health Organization advises a higher cut-off of 70 micrograms per litre in adults and 30 in children, precisely because a recent bug can flatter the number.

None of which is cause for alarm. One figure is the start of a conversation rather than a verdict, and "your result was normal" is not quite the same sentence as "nothing is going on".

Who Actually Needs to Think About Iron

Most people, not at all. A few groups reasonably might, less as a risk register and more as useful self-knowledge. Menstruation and pregnancy are the situations that raise requirements most, which is why European reference values are set higher for premenopausal women than for anyone else. Heavy periods are the usual explanation in women of reproductive age, and are routinely underestimated by the person having them. Plant-based eaters are worth adding, since iron from plants goes less far. Endurance athletes too.

Outside those groups, if you feel well, treat this as trivia and enjoy your day.

The Practical Bit: Iron From Food

Pleasingly low effort, this part.

Iron turns up in two forms. Haem iron, from animal foods, absorbs easily: red meat, lamb, pork, poultry, fish. Non-haem iron, from plants, absorbs less readily: beans, peas and lentils, dark green vegetables such as spinach, kale and broccoli, nuts and seeds. On quantities, the European Food Safety Authority sets the reference intake at 11 mg a day for men, 16 mg for premenopausal women, and the same as men for women after the menopause.

How much of that iron you actually take up varies enormously, and this is the bit most people never hear. Evan puts the range at anywhere from 2 to 40 per cent of the iron on your plate, depending on the food itself and on what else is in the meal alongside it. So the company iron keeps matters nearly as much as the amount.

Absorption is where small habits earn their keep.

  • Put something with vitamin C next to plant iron. Peppers, tomatoes, a squeeze of lemon over the lentils, a glass of juice with your fortified cereal
  • Eat plant and animal sources together if you eat both, because animal protein foods help you absorb non-haem iron
  • Shift the tea and coffee away from mealtimes. Tannins bind iron. Between meals rather than with them, problem solved
  • Go easy on bran cereals alongside your iron sources, since phytates work against absorption in much the same way

Iron also does not work alone. Absorbing it, moving it about and putting it to use involves vitamin C, folate, B12, copper and vitamin A, which is a decent argument for eating with some variety rather than fixating on a single mineral. Build that from food, or find a daily foundational blend an easier way to keep it steady. Either is fine.

And If You Are Worried

Please do not panic-buy iron tablets off the back of a video. Iron is not a supplement to take speculatively, and more is not better in any straightforward sense. Iron can accumulate, and a ferritin sitting well above the normal range is a problem of its own rather than a superior result, with the liver among the organs that can be affected. A high number is not a high score.

If you are properly wrung out, your doctor can check. That is the reassuring bit: this is one of the easier things in health to simply find out, so it does not need to take up residence in your head.

If your stores do turn out to be low, it is a fixable and unremarkable thing, and your doctor will handle the how. Restocking runs over months rather than days, which is worth knowing so you do not decide after a week that it is not working.

Ferritin is your iron reserve, and it is one of the more interesting numbers on a blood test because it moves before the others do.

"Ferritin face" is decent physiology in a slightly silly hat. The mechanism exists. The diagnosis does not. Dark circles are mostly heredity, pigment, thin skin and age, with a late night on top.

Eat with some variety, park something citrusy beside your lentils, keep the tea away from your dinner. If you feel well, you almost certainly are. Foundations first, then get on with your day.
